CONNECTED COMMUNITIES PROGRAM
OVERVIEW
Peak Agile assisted the Massachusetts eHealth Institute (MeHI) with the development and deployment of their Connected Communities Program. Serving as Senior eHealth Advisor and acting Connected Communities Program Manager for MeHI, Peak Agile provided strategy and advisory and project management services to lead MeHI’s Connected Communities Program and Implementation Grant Program. Peak Agile was responsible for developing the Community Engagement Plan and Community eHealth Templates which MeHI used organize program areas, sharpen objectives and establish guidelines for program and community activity. Peak Agile also provided grant management services to support MeHI's eHealth eQuality Incentive Programs.
DETAIL
To assist MeHI better understand the health information technology and health information exchange ecosystem at the state and local level, Peak Agile developed a Community Engagement Plan. The Community Engagement Plan organized the core areas of the Connected Communities program, sharpened objectives and established guidelines. It translated the program objectives into reference items, templates and tactical devices for the MeHI Community Teams to use when conducting community outreach and engagement and synchronized the program team activity.
Peak Agile led the next critical phase which focused on statewide stakeholder interviews with health system executives, providers, staff and HIT leaders representing all sizes, sectors and regions. Peak Agile then facilitated community round-table sessions centered on identifying clinical and business needs, challenges and barriers and HIT improvement ideas. Peak Agile formed the process and tools to assist MeHI with the synthesis of stakeholder responses and community feedback which established focus areas and set eHealth priorities for unique Community eHealth Plans used to shape the goals of the Connected Communities Grant Program and to inform and contribute to a larger Statewide eHealth Plan for Massachusetts.
Peak Agile also served as Grant Manager for a number of MeHI grant recipients focused on improving and expanding health information exchanges and use cases among acute, primary care, behavioral health and long-term and post-acute care sectors. Peak Agile provided client-level coordination ensuring participants remained compliant and on-track with their awards, assisting with the administration of program and grant reporting, transformation plan approvals, milestone tracking and confirmations for fund distributions.
